This form is for requests from Tyne and Wear residents to renew:

  • a CT Pass for people with a disability #
  • an expired CT Pass for older people
  • a CT Pass for older people that hasn’t been used for 12 months
  • a CT Pass for older people who have changed address
  • or replace a lost, stolen, faulty or damaged Concessionary Travel Pass – both for older people and those with a disability

If your Concessionary Travel Pass for older people expires in March 2023 and you haven’t received your new pass yet, please complete this form.

If your Concessionary Travel Pass for people with disabilities expires on 31 March 2023 and you haven’t received your new pass yet, please complete this form.

Please allow 28 days from us receiving your application for your new pass to be issued
